The Program Director supervises assigned homes, Group Home Managers, and leads a person-centered team which provides services and supports the individual’s homes.
They are also responsible for ensuring that the Group Homes under their supervision operate within the policies and procedures of Heart to Heart Home Care.
*Essential Job Functions:* 1.
Manages all group home functions and Group Home Managers for the assigned homes.
2.
Responsible for management and implementation of relevant processes, policies, and procedures.
3.
Responsible for company compliance towards processes, policies, and procedures.
4.
Lead, manage, supervise, and train direct supports and other junior employees.
5.
Develops and maintains an interactive relationship with each individual that reflects the values and principles held by Heart to Heart Home Care.
6.
Implements the person-centered plan by identifying and providing support services to each individual living in the group home.
7.
Serves as the Service Plan Coordinator for each individual, overseeing implementation of the Service Plan 8.
Supervises group home staff in accordance with Agency policies and procedures.
Provides on the job and in service training as needed.
9.
Responsible for managing staffing needs to effectively reduce consistent overtime.
10.
Submits weekly schedule to supervisor.
11.
Participates in all management team meetings and required in-service training sessions.
12.
Provides planning and input to the budget development process and monitors expenditures to operate within approved budget.
13.
Provides direct care to individuals as needed, including the provision of emergency coverage to the group home.
14.
Medications are usually administered by Community Support Staff.
In their absence, when necessary, the Program Director may administer medications and records in compliance with Heart to Heart Home Care policies and procedures which conform to DDD guidelines.
15.
Conducts audits weekly on assigned programs and send reports to immediate supervisor.
16.
Complete weekly and monthly Smartsheets.
17.
Completes Therap audits on a daily basis to ensure all documentation is present and in accordance with Licensing and DDD standards.
18.
Completes all DDD required trainings and continues to improve skills through trainings required by Heart to Heart Home Care.
19.
Cooperates with Heart to Heart Home Care and Department of Human Services staff during any inspection or investigation 20.
Attends all IDT and ISP meetings for consumers in their homes 21.
Provide QA with weekly and monthly documentation as requested 22.
Responsible for timely submission of UIR information.
23.
Ensures all staff remain in full compliance with DDD and H2H Policies.
24.
Duties and tasks as assigned.
Job Type: Full-time Pay: $65,000.
00 - $70,000.
